Before Filling Out This Form...

You will need to create your son's SAT or ACT account before we can register him for the test. Please do this before the Registration Deadline. You can find information on creating your son's SAT or ACT account on this page.

Test Dates and Home Passes
When deciding when you would like your son to take the ACT/SAT, please take into consideration when he has home passes planned. We are unable to change test dates if it is scheduled during a home pass. By asking us to register your son for the ACT/SAT, we assume that you have researched his home visits and made certain there are no scheduling conflicts. We will notify your son’s therapist of your desired test date.
